INSTRUCTIONS TO THE CANDIDATES :
- Application form for MBBS, BDS, BPTh, B.Sc. Nursing, Post Basic B.Sc. Nursing, B. Pharmacy, B. Sc. Biotechnology, B. Sc. Microbiology, B. Sc. Honors Biotechnology) & B.Sc. in Allied Health Science (Perfusion, Neurophysiology and Imaging Technology) & Diploma in Skill Programmes,
- M.Sc. (Medical) Microbiology, Masters in Public Health, M. Pharmacy, M. Sc. Microbiology, M.Sc. Biotechnology, M.Sc. Pharmaceutical Microbiology, PG Diploma in Skill Programmes & PG Diploma in Hospital Administration and Management & Fellowship. should be submitted along with Degree certificate fee, Rs. 2000 for conferment in person and Rs. 2100 in absentia.
- Application form for Post Graduate Degree in MD/MS, MDS, MPTh, M. Sc. Nursing, M. Sc. Nursing (NPCC), M.Ch. Neurosurgery, M. Ch Urology & DM Cardiology & Ph.D. should be submitted without any Degree certificate fee but should submit Rs. 100 in case of absentia.
- Any complaint regarding non receipt of the Degree certificate or correction in name, class, subject, year of passing etc will be entertained within a period of three months from the date of concerned convocation. No complaints will be entertained after the specified period.(After 30 September 2026)
Mandatory Enclosures :
- Attested Xerox copy of Passing Certificate.(UG/PG)
- Attested Xerox copy of Internship Completion Certificate. (For MBBS, BDS & BPTH)
- Attested Xerox copy of Provisional Degree Certificate for Ph. D.
