Application Form for XIIth Convocation

INSTRUCTIONS TO THE CANDIDATES :
  • Application form for MBBS, BDS, BPTh, B.Sc. Nursing , Post Basic B.Sc. Nursing, B. Pharmacy, B. Sc. Programmes & Diploma in Skill Programmes should be submitted along with Degree certificate fee, Rs. 2000 for conferment in person and Rs. 2100 in absentia.
  • Application form for Ph. D, M.Ch. Plastic Surgery, M. Ch. Urology & DM Cardiology, Fellowship, Post Graduate Degree in faculty of Medicine, M.Sc. Medical Microbiology, M.Sc. Medical Physiology, MDS, MPTh, M.Sc. Nursing, M.Sc. NPCC., M. Sc. Microbiology, M.Sc. Biotechnology, M.Sc. Pharmaceutical Microbiology & PG Diploma in Skill Programmes should be submitted without any Degree certificate fee but should submit Rs. 100 in case of absentia.
  • Any complaint regarding non receipt of the Degree certificate or correction in name, class, subject, year of passing etc will be entertained within a period of three months from the date of concerned convocation. No complaints will be entertained after the specified period.
Mandatory Enclosures :
  • Attested Xerox copy of Passing Certificate.(UG/PG)
  • Attested Xerox copy of Internship Completion Certificate. (For MBBS, BDS & BPTh)
